This form is for citizen's who have received a Notice of Nuisance Administrative Citation and wish to schedule a hearing with the Administrative Law Judge.

Please be aware that requesting a hearing does not waive any civil fines. However, completing this form for an administrative hearing within 10 days of the notice will stay a late fee until you can speak with the Administrative Law Judge.

All hearings are held in the Council Conference Room located at 56 North State Street, Orem, Utah 84057. Please enter through the North entrance of the City Center Building between the library and the Council Chambers.
