FINAL EVALUATION REPORT

 

The Evaluation Report is due within 60 days of the completion of the project, and no later than August 31st, 2023. Please return the completed report to CARE Program, c/o City of Orem, 56 N. State Street, Suite 101, Orem, UT 84057.

You may use up to three pages to answer the following questions:

1. If you are using CARE funds for general operating expenditures:            

          a. Describe what challenges and/or opportunities you faced, and what plans your organization had for addressing them.

           b. State your organization’s goals for serving the community in the City of Orem. How are CARE funds helping you to accomplish these goals? Who, where and how many constituents did you hope to serve/reach during the grant period? What were the outcomes? How are you measuring the success of your efforts?

2. If you are using CARE funds for a specific project:

          a. Describe what has been accomplished during the grant period, citing any particularly noteworthy achievements, problems, or changes to the project.

          b. Describe the impact this project is having on the community. Who, where and how many constituents did you hope to serve/reach during the grant period? How did CARE funds help accomplish these goals? What were the outcomes? How did you measure the success of your efforts?

          c. Are other non-CARE monies being used on the project?  How much? What is the percentage in terms of the project’s budget?

Attach examples of how the CARE Program was acknowledged in programs, brochures, advertisements, etc.

Include a spreadsheet or document detailing how your CARE grant funds were utilized during the grant year (July 1 - June 30).  Make sure all items are qualifying expenditures and allowed under the CARE guidelines.  If CARE funds were used for personnel, detail how funds were used by position, including salary and benefits for new positions and any increases to salary or benefits for existing positions.
