City of Orem CARE Program
Cultural Arts Major Grant Application
The City of Orem CARE Application, due by 5:00 PM on February 28, 2025, covers activities between July 1, 2025, and June 30, 2026, which would be supported by CARE grant funds.

If you are a new applicant to the City of Orem CARE program, you may be asked to fill out an additional questionnaire for us to better understand who you are and your operations.
As a note, one of the limitations of this application system is the inability to save & continue your application. To help make this easier, here is a document that has all of the questions you will need to answer. You can use this document to answer the questions offline and then copy/paste them into the application when you are ready to submit.

Is your organization located within the City of Orem (this is required for all major grant recipients)?
Is your organization a cultural organization and does your organization currently maintain nonprofit status? If not, your organization is not eligible for CARE funding.
Category of Discipline (select all that apply)
Is your organization affiliated with a college, university or educational institution?
If so, does the institution receive more than 50% of its funding from state funds?
Financial Reports: Organizations requesting amounts of $10,000 or more are required to attach financial reports for their organizations prepared by a Utah licensed certified public accountant/firm according to the following schedule:
a. $10,000 to $99,999 – a compilation report for the most recently completed fiscal year;
b. $100,000 to $499,999 – a review of the most recently completed fiscal year plus a compilation report for the previous two fiscal years;
c. $500,000 or more – a review for the most recently completed fiscal year and the two previous fiscal years.

Confidentiality of Financial Information: Under Utah Code Annotated 63-2-304(2)(a)-(b), your organization’s financial information may be classified as protected and kept confidential by the City, if you provide the following claim of business confidentiality and provide the following statements of reasons supporting the claim of business confidentiality:
