City of Orem CARE Program

Cultural Arts Major Grant Application

The City of Orem CARE Application, due by 5:00 PM on February 28, 2025, covers activities between July 1, 2025, and June 30, 2026, which would be supported by CARE grant funds.

If you are a new applicant to the City of Orem CARE program, you may be asked to fill out an additional questionnaire for us to better understand who you are and your operations.

As a note, one of the limitations of this application system is the inability to save & continue your application. To help make this easier, here is a document that has all of the questions you will need to answer. You can use this document to answer the questions offline and then copy/paste them into the application when you are ready to submit.

Executive Summary - In order to maintain perspective while reviewing your application, please provide a short summary of who you are, why you are applying, and what you propose to do with the funds. This section will be used in summary report to our CARE Commission (limited to 1200 characters).

Financial Reports: Organizations requesting amounts of $10,000 or more are required to attach financial reports for their organizations prepared by a Utah licensed certified public accountant/firm according to the following schedule:

a. $10,000 to $99,999 – a compilation report for the most recently completed fiscal year;

b. $100,000 to $499,999 – a review of the most recently completed fiscal year plus a compilation report for the previous two fiscal years;

c. $500,000 or more – a review for the most recently completed fiscal year and the two previous fiscal years.
