As soon as possible, complete and submit the forms found here. Please read all informtion carefully and follow all isntructions. Once we have recieved the completed forms, we will reach out to you with further information abour reporting to work.
a. Employment Eligibility Verification Form: fill in all required information on the first page and check the box in the middle of the form that applies to you (there are 4 options; only one should be checked). The second page is for the employer to fill out, so you do not need to do anything on this page. You will not need to upload your ID for this form, but you will need to bring that in to the HR office when you start working. For your information and convenience, the List of Acceptable Documents form is included on page three; you will need to bring either a List A document by itself, OR a List B document together with a List C document (all documents must be valid and unexpired) to the HR office on (or by) your first day of work.
b. Employee EEO Status Declaration: we are required to report on this to the EEOC, but this is optional to complete. We do need your name and date, and then if you choose to complete the form, simply check the two boxes that apply to you.
c. Employee Information Sheet: Please complete all fields and check all four acknowledgement boxes (these boxes state that you have received the attached documents and have watched the video linked to above). In the address boxes, we need full address including city, State, and zip code.
d. W-4 Employee’s Withholding Certificate: Please complete all fields in step 1 (including checking the box in section 1.c that applies to you). Steps 2, 3, and 4 are optional, and instructions/worksheets that may be helpful in knowing if you should complete these steps are found on the following three pages.
e. Direct Deposit Authorization Form: Enter applicable bank information so that we can set you up for direct deposit of your paychecks.
f. Statement Concerning Your Employment in a Job Not Covered by Social Security: Because the City of Orem is Social Security exempt, you will not see Social Security taxes being withheld from your paycheck; depending on your work history, this may affect what you are eligible for from Social Security in the ways listed on this form. The employee I.D. number that you should enter here is simply your social security number.
g. At this point, click on submit and sign. Once you have entered your signature, you will have to scroll back through all pages of this document and click everywhere that requires your signature to apply that signature onto that specific form.
