City of Orem Police GRAMA Request Form

You may be required to pay a fee to cover the cost of providing these records.

The current fees are:

$25 for each police report

$30 to $45 for photos

$45 for body/dash cam videos for 30 min, $15 an hour for each additional hour spent processing

Photo ID may be required before any requested records are released.

If you choose not to include ID with your submission, any mention of your own personal identifying information in the requested records WILL ALSO BE REDACTED. If you need your own name, address, etc. included in your requested documents then you MUST provide a copy of your photo ID with your GRAMA request
