RUN A CROOK OUT OF TOWN 5K RELEASE AND WAIVER OF LIABILITY

Participant acknowledges that there are risks of personal injury, illness, possible loss of life, and risks of
damage to or loss of personal property, which may result from participating in the Run a Crook out of
Town 5K (“5K”). Participant voluntarily enters the 5K and assumes all of these risks. Participant, and
anyone entitled to act on Participant's behalf, as a condition of entry, agrees to indemnify and hold
harmless the City of Orem its officers, elected representatives, employees, and volunteers (collectively
“the indemnified parties”) from any and all claims and costs, including attorneys’ fees, arising from or in
connection with participation in the 5K. In no event shall the indemnified parties be liable to the
Participant for acts or omissions arising out of or related to the 5K or that Participant’s participation in
the 5K.
Participant gives permission for the use of his/her name and photograph in connection with this 5K and
for the City of Orem’s use thereafter. Participant also acknowledges that he/she has read, understands
and will abide by the rules of this contest.
Participant agrees that Participant’s electronic signature, whether digital or encrypted is intended to authenticate this writing and to have the same force and effect as a manual signature. Electronic
signature means any electronic sound, symbol, or process attached to or logically associated with a record and executed and adopted by a party with the intent to sign such a record, including facsimile or email electronic signatures.
Participant acknowledges that Participant has read and understands the conditions set forth in this Run
a Crook out of Town 5K Release and Waiver of Liability (“Release and Waiver”), and that Participant
agrees to all conditions set forth herein, and that Participant signs this Release & Waiver voluntarily. If
Participant is under the age of eighteen the Participant’s parent or guardian must also review these
items and sign below.
